An evaluation of the Social Determinants of Health Program which was undertaken concurrent with implementation of the Program. The program which commenced in September 2014 and concluded in June 2016 and was established to address the level of disadvantage in Tasmania with the aim of fostering local and sustainable solutions to improve health and wellbeing outcomes for communities.
